Use this form to request records your department has stored in the Records Center. Request types include:
A Request Delivery Report listing all of the documents included in the campus mail packet will be accompany your request. Please sign and return this form to our office immediately.
To return documents, verify that a barcode label is on the back of each document to be returned. Place them in a campus mail envelope and mail to us. Once we have received and returned your documents to their original boxes, you will receive a Checked-in Folder Status Report listing the documents returned and date received.
